Hospice & Palliative Care Services

  • Quality Of Life Phone Consult

This is a phone call to discuss quality of life concerns facing a pet patient. In general terms, Dr. Hayes can help assess factors affecting quality of life, implement an ongoing quality of life evaluation system, and provide support and resources to shepherd a pet family through the end-of-life process.

This rate (tax exempt) includes the first 60 minutes of consultation time as well as time to acquire and review pertinent medical records and history, as necessary.

Not included: the ability to prescribe medications. An in-person visit with Dr. Hayes is required to establish a veterinarian-client-patient-relationship, which is the foundation of care of the pet patient for diagnosing, treating, and prescribing.

Additional Time (Beyond 60 Minutes):

$25.00 each 10 minutes ($150.00/hour)


  • Quality Of Life Home Visit

This is a house call performed to establish a veterinarian-client-patient relationship in order to offer hospice and palliative care services.

Dr. Hayes will obtain all pertinent medical history, perform a complete physical exam, and offer a comprehensive treatment plan to address quality of life concerns affecting the pet patient. Working together with the pet family, Dr. Hayes will help establish goals of care for the pet patient. Dr. Hayes will offer support and education to the pet family to help achieve those goals.

The rates (tax exempt) include the first 60 minutes of the house call, and all necessary and customary patient history research in order to make a complete assessment. All driving mileage is calculated using Google Maps with Melrose, MA 02176 as the base of operations.

Not included: medications or treatments prescribed in the course of developing a pet hospice or palliative care plan.

Humane Euthanasia Services

  • Humane Euthanasia House Call

This is a house call performed to provide humane euthanasia services when the pet family and Dr. Hayes are in agreement that quality of life is no longer at the desired level for the pet patient.

As necessary and appropriate, Dr. Hayes will obtain all pertinent medical history to help inform this decision-making process.

At the house call, Dr. Hayes will perform a physical exam suited to the pet patient’s condition before providing humane euthanasia services.

The rates (tax exempt) include the house call, all necessary and customary patient history research, and all pharmaceuticals needed in order to provide anxiolysis, sedation, analgesia, and humane euthanasia. All driving mileage is calculated using Google Maps with Melrose, MA 02176 as the base of operations.

Not included: after-care elections such as cremation or memorial products and keepsakes (see next sections).

Traditional Cremation Services

  • Traditional Private Cremation

This is traditional cremation of the pet’s remains individually by heat, performed by Final Gift Pet Memorial Center in Middleborough, MA.

The rates (taxable) include transportation (provided by Dr. Hayes) of your pet for cremation services, a complimentary urn of your choosing, and complimentary delivery of the urn with ashes to your local veterinary clinic for pick up.

Not included: urns other than those on the complimentary list, personalization of the urn (engraving or name plate; various prices) or UPS shipping of the urn with ashes to your address ($30.00 courier fee).

Traditional Private Cremation

  • <25 pounds = $340.00 + tax

  • 25 - 49.9 pounds = $350.00 + tax

  • 50-74.9 pounds = $360.00 + tax

  • 75-99.99 pounds = $370.00 + tax

  • 100+ pounds = $380.00 + tax


  • Traditional Communal Cremation

This is traditional cremation of the pet’s remains by heat in a group with other pets, performed by Final Gift Pet Memorial Center in Middleborough, MA. Ashes are NOT returned to the pet owner. Ashes have a final resting place in Angel View Pet Cemetery, Middleborough, MA.

The rates (tax exempt) include transportation (provided by Dr. Hayes) of your pet for cremation services.

Not included: any service that includes any portion of ashes saved or returned to pet owner. For such services, traditional private cremation must be chosen.

Aqumation Services

  • Individual Aquamation

This is cremation of the pet’s remains individually by water, performed by Petrichor Aquamation in Woburn, MA.

The rates (taxable) include transportation (provided by Dr. Hayes) of your pet for cremation services, a complimentary urn of your choosing, one complimentary unique paw print laser engraving on a keepsake stone, a stainless steel tracking tag that follows each step of the process, and complimentary delivery of the urn with ashes to your local veterinary clinic for pick up

Not included: urns other than those on the complimentary list, personalization of the urn (laser engraving +$25.00 plus tax), or USPS shipping of the urn with ashes to your address ($40.00 courier fee).

Individual Aquamation

  • <5 pounds = $295.00 + tax

  • 5-20 pounds = $305.00 + tax

  • 20.1-40 pounds = $340.00 + tax

  • 40.1-70 pounds = $360.00 + tax

  • 70.1-100 pounds = $370.00 + tax

  • 100.1-130 pounds = $400.00 + tax

  • Communal Aquamation

This is cremation of the pet’s remains in a group by water, performed by Petrichor Aquamation in Woburn, MA. Ashes are NOT returned to the pet owner. Ashes have a final resting place scattered around a private lake in New Hampshire.

The rates (tax exempt) include transportation (provided by Dr. Hayes) of your pet for cremation services.

Not included: any service that includes any portion of ashes saved or returned to pet owner. For such services, individual aquamation must be chosen.
